A 64-year-old patient was referred for colonscopy due to hematochezia. Colonoscopy revealed a darkly pigmented sessile lesion originating from the anal canal without an adenomatous pattern. Biopsies were obtained. Transperineal ultrasound scan revealed a homogeneous, hypoecoic lesion with well-defined margins, without vascular signal. Patient underwent a MRI scan that showed a lesion with intrinsic hyperintensity on T1-weighted sequences, consistent with the presence of melanin, and involvement of the muscular layer. Histopathological report confirmed the diagnosis of anal canal melanoma. Following multidisciplinary evaluation the patient was referred for immunotherapy.
