A 79-year-old man with a history of ulcerative colitis and a previous sigmoidectomy for severe diverticular disease presented with abdominal pain and bloody diarrhea.
Colonoscopy revealed a stricture of the colorectal anastomosis that could not be overcome with a pediatric colonoscope, but only with a standard gastroscope.
Endoscopy suspected a pre-anastomotic fistula, which was confirmed by both local instillation of methylene blue and subsequent MRI. (courtesy of Dr Giulio SODANI)
