A 32-year-old patient was referred to the outpatient clinic because of epigastric pain. Ultrasound trans-abdominal scan identified a well-defined subcentimetric homogeneous hypoecoic lesion in the esofago-gastric junction region. Upper GI endoscopy showed a bulging lined by normal mucosa right at the esophagus-gastric junction, gastric side. During clinical and ultrasound follow-up, patient was asymptomatic but an increase in the lesion size was reported, so that she underwent endoscopic ultrasound examination EUS. The lesion originated from the muscolar layer and the ultrasound features were confirmed. Hence, a leiomyoma was diagnosed and she continued a regular follow-up with trans-abdominal ultrasound
