A very elderly patient (91-year-old man) was admitted to the emergency room for syncope and melena. After blood transfusion and hemodynamic stabilization, he underwent CT to rule out intestinal perforation. After favorable CT images, he underwent upper endoscopy, which, however, revealed a perforation of the duodenal bulb. He did not survive the acute episode.
